Preparing For Your Visit

Intake Forms

You will receive intake and consent forms via email prior to our visit. It is important to fill them out entirely to set all parties up for success for the visit. Forms must be filled out at least 24 hours prior to visit time. Failure to fully complete forms may result in cancelling or rescheduling your appointment.

Home Visits

In a home visit, I want to work with you in your normal environment, so you do not need to do anything special to your space to prepare! Just be sure to have any supplies that you may need or already be using (nipple shield, breast pump and parts, bottles - ready with pumped milk or formula, pillows for feeding etc.) readily available to make the most of our time during the visit. We can practice feeding wherever you and your baby are most comfortable. For some this is the living room couch, a recliner in the nursery, or a bed.

I will bring gloves, a baby scale and a few other tools that we may need during our visit.

While I love animals, it is helpful to have dogs and cats away in another area of the home. I ask that pets are not in the area during consults for safety and comfort for all parties.

Office Visits

My office is located inside of Holistic Hands and Wellness Gallery located at 4253 Monroe St. Toledo, OH 43606. Please arrive 10 minutes early for your appointment, and have a seat int he waiting room when you walk in. I will come to get you when it is time for your visit.

Please pack your diaper bag as usual, as we like to weigh baby with a clean diaper during the visit. If you need help with your pump, be sure to bring the pump, its cord and all parts to the visit. If your baby is being supplemented or we are working on bottle feeding, be sure to bring a bottle with pumped milk or formula ready to feed.

Virtual Consults

We will conduct our visit via Spruce, a secure and HIPAA compliant platform. You must be on the Spruce App to start the visit. Please be on the app ready for the visit at least 5 minutes before the start time.

It is helpful but not required to have a second adult present for virtual consults to act as a “camera man”. It can be hard to turn point the camera accurately while also handling a baby or a pump.

Cancellation Policy

Cancellations within 24 hours or less of visit will be subject to a $75 cancellation fee. Nourish & Bloom Lactation and Wellness reserves the right to discontinue care with late or multiple cancellations.

Cancellations and rescheduling greater than 24 hours before visit can be accommodated via text, Spruce messaging or email.
